• Teacher Evaluation form (Also called Instructor’s Recommendation or Teacher Report form)
• Mid-Year Grade Report form
• Final Report form

Special Application Forms
• Arts Supplement form
• Athletic Supplement form
• Early Decision Agreement form
• International School Supplement to the Secondary School Report form
• Home School Supplement to the Secondary School Report form
• Peer Recommendation form (Also called Peer Evaluation form)
• Financial Aid form

THE BOTTOM LINE
While each phase of the college admissions process is important, nothing is more important than actually completing the admissions application itself. This is when a student finally makes his/her admissions case. When all is said and done, the best case is letting colleges know exactly who you are as a student and person in as articulate and complete a way as you can. Done correctly, that is what will make you stand out from other applicants.
